Les patients qui doivent subir une intervention ou une chirurgie et doivent cesser de prendre la Coumadine temporairement peuvent \u00e9galement y recevoir un anticoagulant de transition. La clinique offre ses services principalement par t\u00e9l\u00e9phone avec, \u00e0 l\u2019occasion, des consultations en personne avec un m\u00e9decin pour l\u2019administration de l\u2019anticoagulant de transition et le suivi apr\u00e8s un \u00e9pisode aigu de thrombo-embolie veineuse (TEV).<\/p>\n<p>La clinique est \u00e9galement charg\u00e9e de tous les aspects du Programme de traitement des thrombo-embolies veineuses qui, gr\u00e2ce \u00e0 l\u2019\u00e9laboration de politiques, \u00e0 des r\u00e9unions du comit\u00e9 multidisciplinaire, \u00e0 des projets d\u2019\u00e9ducation et \u00e0 la v\u00e9rification des processus, contribue \u00e0 faire diminuer le taux de TEV chez les patients hospitalis\u00e9s.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>La Clinique d\u2019anticoagulation offre aux patients qui y sont dirig\u00e9s une surveillance et un service d\u2019ajustement de leur traitement anticoagulant \u00e0 la Coumadine (warfarine).
