Il vise \u00e0 am\u00e9liorer, chez le patient, la mobilit\u00e9 ainsi que la capacit\u00e9 de communiquer et de r\u00e9aliser les activit\u00e9s de la vie quotidienne de fa\u00e7on autonome.<\/p>\n<p>Les patients et les familles participent \u00e0 l\u2019\u00e9tablissement d\u2019objectifs r\u00e9alistes et valorisants qui les aideront \u00e0 atteindre ou \u00e0 maintenir la meilleure capacit\u00e9 fonctionnelle possible.<\/p>\n<p>L\u2019\u00e9quipe de soins est compos\u00e9e des membres suivants : infirmi\u00e8res, travailleuse sociale\/planificatrice des cong\u00e9s, physioth\u00e9rapeutes, ergoth\u00e9rapeutes, orthophonistes, th\u00e9rapeutes en loisirs, di\u00e9t\u00e9tistes et m\u00e9decins.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Le Programme de r\u00e9adaptation est offert aux patients hospitalis\u00e9s et aux patients externes.
