Ce programme offre des soins 24 heures sur 24 aux personnes qui ont besoin de services m\u00e9dicaux et th\u00e9rapeutiques dans un environnement de soutien.<\/p>\n<p>Le programme aide les patients \u00e0 maintenir leur force, leur \u00e9nergie et leurs capacit\u00e9s fonctionnelles avant de rentrer \u00e0 la maison ou de se rendre \u00e0 leur prochain lieu de r\u00e9sidence.<\/p>\n<p>Le programme est compos\u00e9e d\u2019une \u00e9quipe multidisciplinaire qui comprend les soins infirmiers (IA, IAA et PSSP), les m\u00e9decins, les th\u00e9rapeutes en loisirs, les pharmaciens\/pharmaciennes, les assistants\/assistantes en r\u00e9adaptation et les travailleurs sociaux\/travailleuses sociales. Les patients auront \u00e9galement acc\u00e8s aux services de physioth\u00e9rapeutes, d\u2019ergoth\u00e9rapeutes et d\u2019orthophonistes.<\/p>\n<p>Les visiteurs sont encourag\u00e9s \u00e0 rendre visite aux patients entre 11 h et 19 h. Les partenaires de soins peuvent visiter en dehors de ces heures.
