Family Involvement

Please discuss your wishes with your family. Should you or your family require further information your nurse will refer your request to the Chaplaincy Services or the Discharge Planning/Social Services staff.

Living Wills

Please advise your nurse at time of admission if you have a Living Will (Advance Directive). You may wish to discuss this with your doctor or nurse to make sure your wishes are respected. If you have already signed a Living Will (Advance Directive) or similar document, we encourage you to provide the hospital with a signed copy on admission. This will ensure that your wishes are known. For more information, talk to a lawyer or the Discharge Planning/Social Services staff.

Organ & Tissue Donation

Please advise your nurse at time of admission if you have a signed Organ Donor Card. Each year thousands of Ontarians have a second chance to live life to its fullest, thanks to the generosity of organ and tissue donors, however the need for organs and tissue in Ontario continues to outweigh availability. Power of Attorney

Please advise your nurse at time of admission if you have Power of Attorney.

There are two (2) types of Power of Attorney:

• Power of Attorney for Personal Care allows you to name a person(s) to make decisions about your personal care, including health care, if you are not able to.
• Continuing Power of Attorney for Property allows you to name a person(s) to manage your financial affairs for you under specific conditions.
