Please advise your nurse at time of admission if you have a Living Will (Advance Directive). You may wish to discuss this with your doctor or nurse to make sure your wishes are respected. If you have already signed a Living Will (Advance Directive) or similar document, we encourage you to provide the hospital with a signed copy on admission.
