Kidney Care Clinic

This clinic sees patients requiring monitoring/follow up for Chronic Kidney Disease (CKD), Eprex, Peritoneal Dialysis (PD) or Post Transplant.

The clinic initiates and monitors home PD including teaching, home visits, telephone support and ongoing monitoring in the clinic as well as over the phone when blood work results necessitate changes in prescription.
