Anticoagulation Clinic

Patients who are referred to the anticoagulation clinic receive monitoring/medication adjustment of coumadin (warfarin) therapy. It also provides anticoagulation bridging to referred patient’s who’s coumadin needs to be stopped for scheduled procedures or surgery. It provides phone service mainly with some face to face physician consultations for bridging and acute VTE (venous thromboembolism) follow-up.

The department is also responsible for the VTE Prophylaxis Initiative – which includes policy development, multidisciplinary committee meetings, education and auditing of processes that help decrease the rate of hospital acquired VTE.
