Regional Dementia Care Program (Oak Lodge)

The Regional Dementia Care Program provides comprehensive specialized assessment and treatment of older adults (usually >65 years old) and/or adults with an age-related dementia, which is complicated by behavioural and psychological symptoms, and/or medical comorbidities that exceed the capacity of the community, general hospital, complex continuing care and long-term care system.

  • Alzheimer’s disease
  • Vascular dementia
  • Mixed dementia
  • Frontotemporal dementia
  • Lewy Body dementia

The goal is to facilitate change and improvement through activities that:

  • Improve the patient’s overall quality of life by enhancing/retaining patient abilities;
  • Promote patient, family and care partner engagement;
  • Provide learning opportunities for increased community capacity;
  • Successfully transition the patient to the most appropriate community discharge destination.
